Plan for Pharmacological Therapy

  • For trichomoniasis: A single dose of metronidazole (Flagyl) or tinidazole 2 g stat (CDC, 2021a; Kissinger, 2019).
  • For bacterial vaginosis: Metronidazole (Flagyl) 500 mg orally 2 times/day for 7 days (CDC, 2021b).
  • For vaginitis: Metronidazole (Flagyl) 500 mg orally 2 times/day for 7 days (CDC, 2021b).
  • For candidiasis: Clotrimazole1% cream 5 g intravaginally daily for 14 days; OR Fluconazole 150 mg orally in a single dose (CDC, 2021c).

Laboratory/ Diagnostic Studies

  1. A vaginal pH test to distinguish between the presence of vaginalis (grows in an alkaline environment) and Gardnerella vaginalis.
  2. A PCR assay to identify with conviction the presence of vaginalis.
  3. A whiff test to confirm the “fishy odor” characteristic of trichomoniasis.

Therapeutic/ Nonpharmacological Interventions

  • Do not use soap only rinse with water to clean the vagina.
  • Soaking in a warm bath.
  • Drying thoroughly after washing by patting.
  • Do not use hygiene sprays, fragrances, or powders in the genital area.
  • Use pads and avoid tampons while having an infection.
  • Those with diabetes to keep their blood sugar levels under good control.

Patient Education and Anticipatory Guidance

  • Practicing safe sex (using a condom).
  • Remaining faithful to one partner.
  • Regular disinfection of sex toys.
  • Avoid using multiple douching products.
  • Getting medical help immediately after symptoms like foul smelling discharge are seen.
