Harding et al. (2023) summarize the considerations in the perioperative care of transgender patients. The article notes that this population of patients requires a more nuanced approach to the concept of sex as they may have key anatomical, physiological, and pharmacological changes relevant to healthcare delivery. It particularly draws attention to the use of exogenous hormone replacement within the population of gender-affirming surgery and their impacts on perioperative care. A significant nursing diagnosis among the transgender population is the increased risk of myocardial infarction, a two-fold increase in the risk of myocardial infarction among transgender men when compared to cisgender men and transgender women. Transgender men have a four-fold increase in the risk of myocardial infarction when compared to cisgender women. This increased risk of myocardial infarction reported among the transgender population has been attributed to consequences of hormone interactions and cross-reactions, and impact on societal and economic discrimination. Caceres et al. (2020) note that while there is an increased risk of cardiovascular disease among the transgender population, nursing intervention can be applied to reduce risk and incidence through screening for cardiovascular disease and preparation to address cardiovascular health within the context of other medical interventions that may be concurrently pursued.
