The Social Determinants Of Health Discussion
To prepare:
How would your communication and interview techniques for building a health history differ with each patient?
How might you target your questions for building a health history based on the patient’s social determinants of health?
What risk assessment instruments would be appropriate to use with each patient, or what questions would you ask each patient to assess his or her health risks?
Identify any potential health-related risks based upon the patient’s age, gender, ethnicity, or environmental setting that should be taken into consideration. The Social Determinants Of Health Discussion

Select one of the risk assessment instruments presented in Chapter 1 or Chapter 5 of the Seidel’s Guide to Physical Examination text, or another tool with which you are familiar, related to your selected patient.
Develop at least five targeted questions you would ask your selected patient to assess her health risks and begin building a health history.
How would your communication and interview techniques for building a health history differ with each patient?
Each patient’s communication style and health history-building goals will differ (Coughlin, 2019). For example, some patients may want to share every detail of their health history with you, while others may prefer to withhold certain information. A healthcare provider should adapt his or her communication and interview techniques accordingly to ensure that all of the patient’s health history is collected. How might you target your questions for building a health history based on the patient’s social determinants of health?
Some social determinants of health that may be considered when building a health history with OBGYN patients include: age, gender, ethnicity, socioeconomic status (SES), and environmental exposures (Hamal et al., 2020). The interviewer should target the questions to assess these social determinants of health in order to collect the most accurate and complete health history.
What risk assessment instruments would be appropriate to use with each patient, or what questions would you ask each patient to assess his or her health risks?
Some of the assessment instruments that may be appropriate for use with OBGYN patients include the Prevalence and Risk Assessment of Fetal Alcohol Spectrum Disorders (FASD) tool, the Patient Reported Outcomes Measurement Information System (PROMIS) module for gynecology, and the Female Sexual Function Index (FSFI) (Colalillo et al., 2021). Each of these tools can help healthcare providers better understand the prevalence and risk of certain issues related to reproductive health in women, and can provide clinicians with information they need to better care for these patients. Additionally, the FSFI can help clinicians measure the overall health of women’s sexual function, which can provide them with valuable information about how best to support these patients. Finally, PROMIS can help healthcare providers track patient outcomes and assess how well their care is meeting patients’ needs (Colalillo et al., 2021)The Social Determinants Of Health Discussion . Using these assessment tools together can provide OBGYN clinicians with a comprehensive understanding of their patients’ experiences and potential risks related to reproductive health.
Questions that may be appropriate to ask OBGYN patients to assess their reproductive health and risks include;
- Do you have any current health concerns?
- What are your thoughts on contraception?
- Do you have any sexually transmitted infections (STIs)?
- Are you using effective forms of birth control?
- Do you have a history of unintended pregnancies?
- What are your thoughts on abortion?
- Are you considering having children in the future? What are your concerns about having children?
- Do you have any concerns about your fertility?
These questions will help me to better understand the different patients’ reproductive health and risks, and develop strategies to address any problems that they may have. The Social Determinants Of Health Discussion
