Quality improvement initiatives aim to ensure that health care provided to patients is safe and effective, and delivered in a timely manner. The hospice data activity is a quality improvement initiative that focuses on evaluating the data collected by hospices to improve patient care. Analysis of a Current Quality Improvement Initiative in the Health Care Setting

The quality improvement initiative being analyzed is the use of hospice data in a health care setting. This initiative was prompted by the need to provide better quality care to patients who are in the process of dying. The hospital aims at providing comfortable care to the end of life of patients, and thus its care is not based on urgency or any form of life-saving care. This means hospital treat the symptoms, not the disease. Unlike other units, the patient is a recipient of care, but so is the family. It does this by using a holistic approach, in that they try to address not just physical, but also emotional, psychological, and spiritual needs. With each patient, there is an interdisciplinary team that delivers care at home. When symptoms arise that need more aggressive management than home care can provide, the healthcare organization allows the care providers to do temporary inpatient admissions. Problems that were not addressed during the initiative include the lack of communication between different health care providers and the need for a standardized data collection process. The most successful benchmarks or outcome measures used to evaluate the success of the quality initiative were patient satisfaction surveys, patient outcome metrics, and adherence to national, state, or accreditation standards (Head et al., 2019). There were also interviews done on stakeholders of the hospital to understand more about the hospice data activity. From the interviews, it was determined that the most successful aspects of the quality initiative were patients’ satisfaction surveys and patient outcome metrics. Patient satisfaction surveys showed a marked improvement in how well hospice patients were being cared for, while patient outcome metrics showed a decrease in the amount of pain and suffering patients reported. As a result of improved care among the patients who had been admitted in 2020, the number raised slightly from 351 to 376, and this represented over 97% satisfaction rate. The initiative also met all national, state, and accreditation standards. Requirements for national, state, or accreditation standards include meeting benchmarks or outcomes measures to evaluate, measuring, and reporting quality improvement. The results of the hospice data activity show that by using benchmarks or outcome measures to measure success, the hospice was able to improve patient care while still meeting all national, state, and accreditation standards. All of the team members agreed that the QI initiative was a success and that they were proud of their work. In addition, all of the team members believed that the QI initiative was beneficial for patients and improved the quality of care provided to patients. However, the interprofessional team faced some challenges such as communication challenges and coordination challenges. The communication challenges resulted from the team’s inability to share information effectively and the coordination challenges were due to the team’s inability to manage and coordinate their activities. Despite these challenges, the interprofessional team members were positive about their work in implementing QI initiative and believed that it was beneficial for patients.

During the implementation of this QI initiative, I talked to senior officers such as director of hospice services, chief nursing officer, quality assurance director, and chief financial officer. The director of hospice services helped me to understand the main goal and roles play by different stakeholders during the hospice process. The chief nursing officer provided me with insights on how to improve the quality of care and the quality assurance director offered suggestions on how to better measure QI outcomes. The input from other professionals helped me to better understand the challenges that they faced and how to address them. Recommendations of Additional Indicators and Protocols to Improve and Expand Outcomes of a Current Quality Initiative

Some of the protocol changes that I would make are as follows: making sure that all patient data is collected, organized, and reviewed in a consistent and standardized manner, encouraging the use of outcome measures such as Quality of Life (QOL) measures, conducting regular reviews of the hospice data activity to assess effectiveness, and developing a plan for improving data quality (Antony et al., 2019). These changes would help the interprofessional team to better assess the hospice patients’ progress and to make decisions about how best to care for them.
