I have learned so much in the last several weeks from engaging with patients ranging in age from several days through adolescence to early adults during my attachment in the clinic office. The clinic provided me with an intriguing overall experience. The medical practitioners I met at the clinic were extremely competent and helpful in expanding my knowledge and comprehension of the area and in assisting me in applying what I had learned during the course in a real-life setting. It is exciting when pediatricians can help solve previously unexplored challenges. The clinician was knowledgeable about a wide range of skin disorders impacting juvenile patients. My definition of family and its roles have changed significantly since I began my clinical practicum experience. I have always considered a family to be a group of people related by blood or marriage. However, I have realized that family is much more than that. A family is a group of people who love and support each other, regardless of whether or not they are related by blood or marriage (Thomas, Liu, & Umberson, 2017). I have found that my definition of family has changed in two major ways. Firstly, I now consider my patients’ families part of the patient’s care team. When a patient comes into my office, I always take the time to talk to their family members and get to know them. Secondly, I now see myself as part of the family. I no longer view myself as an outsider coming into the family house to care for them. As I look back on my practicum experience in the clinic, I can critically reflect on my growth and development as a clinician. I have come to better understand the importance of family and family roles. Interaction with diverse families in the clinic has significantly influenced my skills and expertise in family and pediatric assessments. Prior to my practicum experience, I held the belief that the nuclear family was the most important unit in society. I viewed children as their parents’ extensions and saw them needing constant supervision and care. After working with families in the clinic, I have begun to understand that this is not always the case. Many families are composed of multiple generations living together, creating unique family dynamics (Burns et al., 2017). Additionally, I have come to see children as individuals with their own set of needs and desires. Besides, it has taught me that the family is a complex and dynamic unit that must be assessed on a case-by-case basis.  I am now better equipped to assess the family dynamics and provide the best possible care for each child.

  1. Explain how your understanding of culture (both the provider’s culture and that of the child and his or her family) has changed and how it may have influenced the assessments you conducted during your practicum.

In the clinical setting, I was constantly reminded of the importance of understanding the culture of the provider, the child, and his or her family. I was also reminded of the importance of conducting assessments in a culturally competent manner. I also learned that I need to be mindful of the cultural differences that may exist to provide the best possible care for each individual. When working with children and families, it is important to understand their culture.

One way I attempted to understand the culture of a provider was by observing their behavior and interactions with patients. I often identified patterns in how a provider interacted with patients, which helped me better understand their cultural norms. Additionally, I observed how the provider interacted with the child and family members. This allowed me to get a sense of their cultural values and how they may have influenced the assessment I was conducting. However, it is important to note that not all assessments apply to every culture (Handtke et al., 2019). For example, in some cultures, it is acceptable to show emotions while others may not. Additionally, assessments may differ depending on a family’s socioeconomic status. Therefore, it is important to tailor assessments for each individual and culture.

I could meet most of the objectives and goals I stipulated in week one. The first was that my skills, knowledge, and expertise in conducting clinical assessments as well as diagnosing children and their families grew in relation to what I already knew. Also important to me was the question of comfort in pediatric and adult clinical settings.
