SUBJECTIVE:

Chief Complaint: The patient complains of foot pain.

History of present illness:

PMH:

The patient, TJ, has no past illnesses. She records having scrapped the bottom of her right foot while twisting her ankle while tripping. She also reports that they wound will not heal despite being constantly cleaned with soap, water and taking Tramadol 50MG.

Accidents/Injuries:

The patient tripped thereby scrapping the bottom of her right foot and twisting her ankle. When admitted to the emergency department, her x-rays showed no fractures. The patient is now taking Tramadol 50MG. SOAP Note For Foot Pain Assignment Paper

TJ did not provide any account of recent hospitalizations.

FH:

The patient’s father had hypertension and type 2 diabetes. He is deceased after a car accident. Her mother was diagnosed with depression after the death of her spouse. She suffers from high cholesterol, and hypertension. The patient’s sister is also asthmatic while her only brother has no health concerns. Her (P) grandmother has high cholesterol. She is taking blood pressure medication for her hypertension. The patient’s (M) grandfather is deceased resulting from a car accident as well. Her uncle suffers from alcoholism. There is a family history of obesity as well.

Social History

Allergies:

The patient is asthmatic. She is allergic to cats which cause her to wheeze. Her eyes also itch while she experiences asthma exacerbations. She is allergic to penicillin which gives her hives and rashes. TJ denies any allergies to latex and foods.

Current Medications:

The patient takes Tramadol 50MG, Advil 200MG, Prventil (Albuterol Sulfate) Inhaler 90MCG

Review of Systems:

General: TJ was then asked about her constitutional health, to which she states experiencing a 10 LB weight loss in the past month, but no fever, chills, or night sweats. TJ also, reports that the wound does not heal and that it displays redness, white purulent drainage and swelling. Her blood pressure is elevated and her blood sugar levels are uncontrolled. Eyes: The patient has no eye pain, discharge or redness bust has blurry vision at times.

Head: She also reports no headaches or dizziness.

Ears: BF reports no change in hearing, no ear pain, and no ear drainage.

Nose: She has no epistaxis, and no congestion.

Mouth: She experiences no sore throat, and no difficulty swallowing.

Neck: She experiences stiffness when reading, and discoloration.

Chest: BF reports dyspnea, and wheezing.

Heart: She reports no chest pains, and no palpitations.

GI: She has no change in appetite, no abdominal pains, no bowel habit changes, and no emesis.

GU: BF reports no urinary urgency, no dysuria, and no change in nature of urine.

Gyn: The patient states she was experiencing no change in menses, no dysmenorrheal, no vaginal discharge, no pelvic pain and no amenorrhea/menopause.

Musculoskeletal: She reports ambulation difficulties r/t right foot wound. No muscle weakness, or bone issues.

Psychiatric: She has no depressive symptoms, and no changes in sleep habits. Patient denies thought of hurting self or others.

Skin:  Patient reports acne skin breakouts.

Hematologic: heavy bleeding during menstrual cycle

Vitals: Blood pressure 113/78, O² sat 99%, pulse 91, respirations 18, temperature 36.0 C.

General: TJwas an edgy and flushed. She appeared a bit distressed and was smirking at times. Patient was obese. She experienced no distress during the assessment.

HEENT: Cardiovascular: S1, S2, no murmurs, gallops, or rubs. No S3 and S4.

Skin: Moist, mucous membranes, normal skin turgor, no tenting. No lower extremity edema.

Respiratory: Client’s breathing was quiet and unlabored. She could construct full sentences and her respiration was clear to auscultation.

Abdominal: 6 cm scar in RUQ and 10 cm scar at the midline in the suprapubic region. An abdominal assessment revealed no discoloration and bowel sounds were normoactive in all quadrants; no bruits; no friction sounds over spleen or liver; tympany presides with scattered dullness over LLQ; abdomen soft in all quadrants. No masses noted in the LLQ; no organomegaly; no CVA tenderness; liver span 7 cm at MCL and no hernias.

Rectal: No hemorrhoids, fissures or ulceration; strong sphincter tone, fecal mass in the rectal vault.

Pelvic: no inflammation or irritation of the vulva, abnormal discharge, bleeding, masses or growth or tenderness upon palpation.

SHE FURTHER STATES THAT “IT’S LOOKING PRETTY NASTY” WITH REDNESS, SWELLING, AND PRULENT DRAINGAGE PRESENT. VOICES C/O PAIN “7” (SCALE 0-10) TO RIGHT FOOT AND VOICES THAT PAIN WORSENS WITH WEIGHT BEARING ACTIVITIES AND WALKING. History of Present Illness (HPI): MS. JONES STATES THAT SHE SCRAPED THE BOTTOM OF HER RIGHT FOOT AND TWISTED HER ANKLE AFTER TRIPPING WHILE GOING DOWN BACK STEPS. AN X-RAY WAS PERFORMED 3 DAYS AGO IN THE ED, NO FRACTURES NOTED. PATIENT HAS BEEN TAKING TRAMADOL 50MG BID AND KEEPING FOOT WOUND CLEAN WITH SOAP AND WATER. SHE CAME IN TODAY WITH C/O INCREASED PAIN, SWELLING, REDNESS, WARMTH, AND PRESCENCES OF PRULENT DRAINAGE. SHE STATES THAT IT HAS BECOME MORE CHALLENGING TO WALK ON RHIGHT FOOT, STATES SHE FEELS A SHOOTING PAIN WHEN WALKING. SHE RATES HER PAIN 7/10 BUT BECOMES MORE SEVERE WITH WEIGHT-BEARING ACTIVITIES. MS. JONES CONTINUES TO STATE THAT SHE HAS BEEN HAVING SOME INCREASE IN URINATION, THIRST, AND APPETITE. HOWEVER, SHE REPORTS THAT SHE HAS HAD AN 10LB WEIGHT LOSS WITHOUT TRYING.
