Demographics: The client is a 29-year-old single, African American male who is unemployed. He lives alone in one bedroom apartment subsided by the county administration .He seems proficient in English. The patient lives alone in a low income urban city in the North. History of Present Illness: The client is alert and oriented to person, place, time, and situation. He reports that he moved to this apartment complex subsided by the county administration six weeks ago. Past Psychiatric History: The client has been feeling the symptoms of depression since the age of 20. He has had two-inpatient hospitalization for the symptoms of depression. The most recent hospitalization was on 01/20/2017 at the County hospital. He mentioned that he was placed on Lexapro 10 mg every morning and it really worked for him. Patient attended one dialectical behavioral therapy (DBT) class a month ago, but believes it will not help him and the class is “boring.”
Medical History: The client has had two inpatient hospitalization in the past. He does not have any known drug or food allergies. All his immunization are up-to-date. The patient received the influenza vaccination on 01/06/17. He denies having Hypertension and diabetes, head trauma, seizures. He had his annual physical examination on 03/07/2017.Denies any pain, distress or discomfort.
Substance Abuse History: He denies the use of illicit drug in the past.
Differential Diagnosis:
- Mood disorder due to another medical condition-After care review of the individual history, physical examination, and laboratory findings there is no reason to include this as a diagnosis (Association, 2013). - Substance/medication-induced depressive disorder-The patient has a previous substance abuse history that included marijuana and alcohol; however the patient has not abused marijuana in 8 years and only drinks alcohol on occasion. The dosage was chosen because this is medication that is indicated for major depressive disorder. Since the patient is normal weight I chose to start the patient at the lowest possible dose and see if his symptoms are better on this dose. Lexapro belongs to a drug class called selective serotonin reuptake inhibitors (SSRI). SSRI are absorbed slowly and undergo little first-pass effect. All SSRIs affect CYP450 metabolizing enzyme and will affect the metabolism of other drugs metabolized by this system (Procyshyn, Bezchlibnyk-Butler, & Jeffries, 2015).
Referrals with rationales: The client will return to the outpatient clinic as scheduled for a routine follow up in three weeks. Three weeks is enough time to evaluate the course of the medication and to determine if there needs to be any medication adjustments. The patient is referred to Dialectical behavior therapy (DBT). DBT will help the patient learn and utilize coping skills in the community. This can be used in addition to the pharmacologic intervention.
