Mr. Juan Duran is a 60-year-old patient who has been coming to the Diabetes Clinic at the VA for a few months. Case Study Reflection

The case study revolves around the 60-years old Mexican American man, Mr. Duran, who has been diagnosed with diabetes over the last ten years. Other prevailing conditions include poor vision and no sufficient knowledge of the insulin syringes. After Jim Carlson finds him wandering along the hospital environment, he asks Jenny, a Diabetic Nurse Educator, to book Mr. Duran for an unscheduled appointment. While assessing the patient, Jenny realizes that Mr. Duran has recently lost his daughter, and his wife has limited English since, at home, they speak Spanish, thus requiring him to take care of his daughter’s child, thus adding up to his diabetes stressor. In that case, the most appropriate theories for addressing Mr. Duran’s case include Orem’s Self-care deficit theory and Neuman’s System model.

Orem’s theory suggests that patients must maintain self-care independence during their recovery. The theory stresses patient education, culture, support, diet, environment, and prevention needs. Hence, considering the current state of Mr. Duran, Orem’s self-care deficit model is appropriate to increase his optimal care in managing his diabetic condition at home. Neuman’s system theory presents the patient as resembling an open system that reacts to. Therefore, Neuman considers various patient variables like the spiritual, developmental, sociocultural, psychological, and physiological variables meaning that the patient system involves basic structures whose protection relies on resistance lines. Orem’s theory comprises of three-related sections: theory of self-care, self-care deficit, and nursing systems. Orem’s theory’s main aim is for the patients to induce themselves into activities that will improve their overall life, well-being, and health. Regarding Neuman’s theory, the main factors to address regarding how Mr. Duran and his wife need to address the prevailing diabetic condition, grief, and raising a young child. That can involve a psychiatrist who can engage Mr. Duran and his wife in a local support group to interact with others who may have lost their loved one and how they coped with the situation.

Adhering to medication and management of diabetic conditions vary from person to person, thus validating the use of Orem and Neuman’s theories. Hence, during my nursing practice, I have encountered a case where the patient had Type 2 diabetes that led to the development of Dipsogenic diabetes insipidus. The main management of the disease involves administering desmopressin in small amounts to help increase urination frequencies at night. Hence, Orem’s self-care deficit theory was necessary since dealing with young people can be a bit hectic, and it was efficient through an endocrinologist who trained the patient on the dietary needs.
