B2: Development of the Action Plan

As the project manager, I planned to gather input on developing my action plan by conducting a series of meetings with the project team. The project manager informed team members about the agenda (major creation of the action plan tool) and provided a brief description of the significance of the action plan and how to create it. According to Sipes (2020), the action plan provides the basis for controlling and monitoring the project because the PM can refer to it and note schedules, major tasks, and deadlines.  I gathered input relevant to developing the action plan through collaboration with the project team. The team met to create the action plan using the best practice informed by previous research.

I used the nominal group technique to reach a consensus among project team members whereby the team proposed action items, discussed them, and ranked top ideas. The team would then vote on top action items, which were then included in the action plan template that would be used to monitor the progress. According to Mason et al. (2021), the best practices involving the nominal group techniques to reach a consensus among members of the project team ensures that everyone’s idea is considered. Input from members was significant in identifying the tasks, determining expected completion data as outlined in the degree schedule, and assigning tasks to a responsible person.

During the meeting, the team considered planning steps to ensure actions documented in the template are thorough. The key steps to planning our action items included planning the schedule management, defining specific tasks, sequencing tasks, estimating each task’s duration to match the degree program schedule, and developing the schedule. To determine the five-high level action items, the team considered all major activities of the HIP, discussed them, ranked them, and voted top-ranked choices for inclusion in the tool. The responsibility was assigned to the project manager and project team who will evaluate the project and prepare a final report. The target date of November 15, 2020, was in line with the degree program schedule that required the final phases of the HIP to take four weeks until mid-November 2022. Overall, input from every team member was significant to help complete the action plan. By documenting my findings and sharing them with the project team, I created a shared understanding of the project’s schedule, objectives, and strategy.

C1: SDOH and Sociodemographic Characteristics

The social determinants of health (SDOH) are the conditions in which people are born, grow, work, live, and age and the wider set of forces and systems shaping how people’s lives unfold. They are also the root causes of ill health and disability, disproportionately affecting disadvantaged groups. The SDOH of geriatric patients admitted to the hospital is divided into five categories: social and Community Context, economic stability, Education, Health and Health Care, Neighborhood and Built Environmental conditions influencing health outcomes.

The target population for my project is geriatric patients admitted to the hospital.

Economic Stability

According to survey findings by Kurnat-Thoma, Murray & Juneau (2022), 30% of the sample population of older adults had income levels at ≤130% of the federal poverty level.  Besides, the study found that 73% of the sample population did not own a home, indicating the poverty levels among older people. According to Zhang et al. (2019), more equity through pensions and healthcare coverage in older adults is a significant leveler that reduces the socio-economic differences in health. The authors highlight that socio-economic resources are significant in alleviating health inequalities that exist among geriatric patients of different socio-economic backgrounds.

Social and Community Context

More than half of older people (56%) are married or live with their partners while 26% live alone (Kurnat-Thoma et al., 2022). Only a few older people (10%) receive community meal assistance. In other studies, older people are beneficiaries of social support, social cohesion, and other social resources, which reduce inequalities in health (Zhang et al., 2019). Social cohesion refers to the social bonds between members in the same neighborhood.

Education

Among older people with age above 65 years, 30% have less than a high school education (Kurnat-Thoma et al., 2022). According to Tzeng et al. (2021), geriatric patients with more than a high school education have a higher likelihood to engage in healthy lifestyles than their counterparts.

Health and Health Care

Less than half (39%) of older people have Medicaid or Medicare health insurance as their only insurance scheme. Only 5% of the older people have had more than two hospitalizations in the past years while 54% have had more than four outpatient visits in the past twelve months (Kurnat-Thoma et al., 2022).  Among the older people in the US, 60% have 1-2 comorbidities, while 12% have more than two comorbidities. The incidence rates of the lifestyle comorbidities are as follows, diabetes mellitus (25%), arthritis (49%), coronary heart disease (10%, stroke (8%), and congestive heart failure (8%). Neighborhood and Built Environmental Conditions 

Most geriatric patients reported favorable neighborhood and built environment that promotes safety and health. However, some African-Americans, Hispanics, and older people of lower socioeconomic status lack access to healthy foods and places of exercise due to their disproportional concentration in neighborhoods with worse environments (Ryvicker & Sridharan, 2018). The authors highlight that safe streets, neighborhoods, walkways, traffic, and recreational spaces promote physical and cardiorespiratory fitness

C2:  Benefits of the HIP for Target Population

The main goal of the current healthcare improvement project on fall prevention is to reduce the number of falls among elderly patients in the geriatric unit. The project team comprising nurses and clinicians will implement multimodal interventions depending on the patient’s needs and the results of the fall-risk assessment. Therefore, the interventions will include educational programs that teach patients how to avoid falls, modifications to the environment (such as installing grab bars in the bathroom), reviewing medications, and providing support services (such as assistance with activities of daily living).

The fall prevention program in the geriatric unit would affect patient outcomes by reducing the number of falls and the severity of injuries sustained in falls. The program would also increase the patients’ confidence in their ability to stay safe when walking and using pathways, reducing the number of emergency department visits for falls. In addition, the program would educate on healthy aging and how to prevent falls, which could lead to better control of chronic diseases such as diabetes and hypertension in the elderly population.

Overall, the fall prevention program is designed to help elderly patients stay safe and independent in the hospital setting and prepare them for a safe home stay after discharge. For example, a patient with a high risk of falls due to a certain antihypertensive regimen and poor muscle strength will benefit from medication review, change of therapy, and exercises to keep them safer and confined while walking or locomotion. The project team acknowledges that each individual might respond differently to fall prevention measures, so it would be important to assess the need for different or multidimensional interventions and identify what works best for each patient.

D2:  Positive and Negative Forces

Force Field Analysis helped identify three positive forces supporting the implementation of fall prevention interventionism in the geriatric unit. The three challenges to the project implementation were the supportive hospital administration, the availability of strong clinical evidence to support its efficacy, and a dedicated team of nurses and staff willing to support the project. Clinical evidence supports the implementation of the interventions, and the hospital’s administration is supportive of facilitating success. Thus the project manager will plan the implementation of the project to include comprehensive intervention strategies within the department. . The hospital faces increased patient falls among elderly patients admitted to the geriatric unit. The trend is unacceptable and requires implementing an effective solution to reduce the incidences, prevent injury, improve patient outcomes and reduce the cost of care, among other outcomes. There is a significant need to implement effective interventions, pushing the project manager to design, plan, execute and evaluate this HIP. The availability of supportive administration will help plan for project implementation by providing confidence about the significance of the interventions to hospital management.

Regarding the negative forces, the project’s success may be limited by the high cost of implementing it, making it difficult for the hospital to justify its investment. The estimated budget requires the funding of staff education, preparation of teaching materials, and the payment of salaries to members of staff involved in different roles. Thus, when planning for the implementation, the project manager and team will review the budget, seek funding and stick to budgeted costs to facilitate success. Another force against the project is the resistance from some members of the geriatric unit staff, who feel that the project is taking away their authority and autonomy. Such members will not receive the project well because they will view it as a nuisance or invasion of their privacy. The PM will also plan to provide incentives and seek funds to pay salaries for care delivered by healthcare workers chosen to deliver care to the target population during the project.

Several strategies can be used to overcome the negative forces that I identified. To overcome resistance from members, I will communicate the benefits of fall prevention and gradually implement the project. Clear communication and effective project implementation can help overcome resistance to change and ensure the project is completed on schedule. Educating staff about the importance of fall prevention and providing them with the necessary training. Staff members may be more willing to accept changes and invest in the project. Gradual project implementation can help minimize resistance to change and ensure staff members accept it. To reduce the project’s overall cost, the project manager will identify cost-effective ways to implement the project, such as using volunteers or technology solutions. With that said, the project would run at a lower cost because it does not rely on full-time staff.

In addition, the project manager would ensure effective project implementation to help overcome the time constraint. Using a skilled team to manage and implement the project, the PM would minimize delays in its completion. With these strategies in mind, the hospital should continue investing in fall prevention initiatives as they will save lives and improve patient safety. Additionally, using a skilled team to manage and implement technology solutions may help reduce the time required for project implementation. A skilled team would quickly assess patients, plan care, intervene and evaluate. Utilizing technology such as the EHRs will help track progress and measure results hence helping in evaluating implemented fall prevention measures. With these strategies in place, the HIP will likely achieve its goals of reducing the number of falls and injuries among elderly patients.

Specifically, the team developed the Gantt chart in a stepwise approach that involved identifying tasks, assigning start and end dates, assigning tasks to team members, adjusting tasks as necessary, and documentation. The first step in creating a Gantt chart was the identification of tasks. The project team identified the tasks necessary for completing their project and organized them into rows and columns on the Gantt chart. The tasks were chosen because they were most significant for the different phases of the healthcare improvement project. The team assigned the tasks to specific people and documented their initials in the chart. When developing a Gantt chart, a few factors are considered when deciding who to assign specific tasks. First, the team considered the skills and experience of each team member and matched the task to their specific strengths. Effective communication is key to ensuring that my project runs smoothly and succeeds. Also, it keeps everyone up-to-date on progress and changes and enhances the prompt management of all concerns. As the project manager, I must maintain effective communication throughout the project. This task requires the submission of your entire HIP paper template, including the “Healthcare Improvement Project Implementation Plan” section of your HIP paper, which you will be developing in this performance assessment and consists of the following subsections:
