Case Study

The patient is a 68-year-old male with community-acquired pneumonia for the past three days. He has a history of COPD, HTN, Hyperlipidemia, and diabetes. He has been on empiric ceftriaxone 1g IV qday and azithromycin 500mg IV qday both for three days. He is currently showing improvement from his status at admission with decreased oxygen requirements but has poor appetite, nausea, and vomiting. He is 5’8” tall and weighs 89kg. His vital signs have improved from admission but some still not within normal ranges; temp 100.9, BP 136/70, HCO3 30, glucose 143 and has increased WBCs. His chest X-ray shows right lower lobe infiltration and heavy growth of streptococcus pneumonia.

Patient’s Health Needs

Since the patient has a history of COPD, HTN, dyslipidemia, and diabetes, he needs proper education on disease control, prevention, and management. He should be educated on the strategies for the management of COPD to prevent exacerbations such as quitting smoking, avoiding triggers such as grass and dust, getting his annual flu shots, and using his medications as prescribed (Halpin et al., 2021). He should also be educated on proper ways to control his blood pressure like adopting a healthy diet including low salt and fat with lots of fiber, fruits, and vegetables. He should also do exercises to help reduce his weight. Treatment Regimen for the Patient

To manage the CAP, the patient will need streptococcus susceptible antibiotics. Since the patient is immunocompromised and is allergic to penicillin, the best antibiotic is levofloxacin. This is the first line of therapy for community-acquired pneumonia in the ward (Welte, 2021). The patient will also need short-acting bronchodilators for COPD exacerbation like salbutamol nebulization. He also needs oxygen administration. The patient is also hypertensive and will need antihypertensive medications to lower his blood pressure. I will give HCTZ and Enalapril. Hydrochlorothiazide is a thiazide diuretic with a fast onset of action and will lower blood pressure through the excretion of excess water (Wijdicks & Clark, 2018). Enalapril is an angiotensin-converting enzyme inhibitor that causes the relaxation of blood vessels and a reduction in blood pressure.

Patient Education Strategy

The patient has many medical needs, and he seems to be having a hard time managing himself. We can adopt a teaching strategy that has the recommendations written down for him so that he does not have to remember everything off head, which is easy to forget. I would recommend written educational materials as my education strategy so that the patient may have his instructions printed out for reference.
