The prevalence rate of elderly patients with dementia may result in a health crisis in the future. Alzheimer’s disease is a condition whereby the brain causes problems in memories and behaviors and it continues to deteriorate as it gradually progresses. The main risk factor associated with Alzheimer’s condition is age. This paper aims to discuss a case study of an elderly with Alzheimer’s disease and its treatment options.
Summary of the Case Study
The case study involves Mr. Akkad, a 76-year-old Iranian male who was brought by his eldest son to the facility. The son reported that Mr. Akkad’s behavior had changed over the past two years, which included his memory loss, challenges to identify appropriate words when conversing, apathy, and personality changes. From the son’s report, it was identified that his behavior change was not associated with any organic disease processes. A personal examination was conducted on the patient confabulation was identified and a deficit in orientation, attention, and recall. The results showed moderate dementia, which is a neurocognitive disorder caused by Alzheimer’s disease.
The Decisions and Supporting Evidence-based Literature
The first decision made will be administering a dosage of Donepezil 5mg at bedtime. Research on the effectiveness of donepezil in the treatment of Alzheimer’s disease has been ongoing for decades. The drug is an acetylcholinesterase inhibitor, which works by increasing acetylcholine levels in the brain which is usually caused by the reduced functioning of the cholinergic neurons (Li et al., 2018)Alzheimer Disease And Treatment Assignment. A research trial on the effects of donepezil on Alzheimer’s patients showed that the medication is effective in the management of behavior, cognition, and functioning. Therefore, since the patient had a behavioral change in his personality, memory, and utterances, the decision would help manage and improve his condition.
The second approach would involve cognitive behavioral treatment; which research has identified to be an effective method early stages of Alzheimer’s condition. Evidence has shown that psychosocial approaches are effective in the management of behaviors associated with dementia (Spector et al., 2015). Thus cognitive behavioral therapy would have supported the change in behavior of the patient. Through immunotherapies and drug administration, a trial has shown that anti-amyloid agents and Aβ monoclonal are effective disease-modifying agents. Therefore, using this approach would prevent further advancement of the condition of the patient.
Expectations on the Decisions
The first decision involving the use of donepezil 5mg during bedtime is expected to bring about symptomatic treatment. The three approaches chosen for the patients would focus on managing behavioral symptoms, maintaining mental functioning, and reducing the progression of the disease. All the decisions supported the expectations. The results of the decision in the exercise aimed to manage the symptoms which were not different from one of three decisions that involved administering donepezil 5mg during bedtime.
