In my practice, I have discovered that ensuring improved public health may involve focusing not only on individual health status but also focus on a population health approach. Population health may face challenges and significant changes that affect different populations. Examples of population health concerns may include chronic diseases, heart diseases, pandemics, and diabetes among others (Ariosto et al., 2018)Approach To Addressing Health Concerns Assignment. In my community, diabetes is a major population health concern that must be addressed to improve public health. Therefore, it is important that stakeholders involved in transforming national health policies address population health concerns.

In my practice, I have identified my strength in providing community-based care as an approach to addressing health concerns. It is out of this, that I decided to have a Community diabetes education program that will educate the community population on the management and prevention of diabetes condition (Powers et al., 2020). I have also Identified that I have a weakness in advocating for policies that promote public health. Policies help improve quality care and patient health. I will join a nursing organization where I can collaborate with other nurses and legislators on policy-making to address such concerns.

An additional resource I have identified is integrating risk management strategies in healthcare to aid in optimizing population health outcomes (Liu et al., 2021)Approach To Addressing Health Concerns Assignment. For example, educating the community on different strategies for the management and prevention of diabetes helps reduce complications associated with the disease. It also helps reduce morbidity and mortality rates caused by diabetes.

Nurse leaders are an important and internal part of advocating for population health initiatives. They are at the most time engaging with patients and other nurses and therefore, resonate with various health concerns affecting different patient populations. Therefore, nurse leaders must be incorporated into policy-making to address population health concerns.
