The change proposal involves community-based education on diabetes. To ensure that the project meets expected outcomes, it is important to plan and implement an evaluation plan. Evaluating a program is essential in judging its effectiveness and impact on the target population. It also ensures that stakeholders remain accountable for resources and help identify ways that can improve the project (Adab et.al., 2018). When evaluating a project, it must answer the question of when who and how, and what. Therefore, project evaluation is a process that goes beyond data collection and helps establish the merit and worth of an intervention.

Collecting data is a crucial part of the evaluation process. The interviews will involve engaging with identified individuals to help identify the benefits of the program (Axelin et al., 2020). The data will also be collected using group assessments in particular community forums where the beneficiaries will give information on how their health has improved after attending the community education program.

The project will involve diverse team members and therefore, those involved in the evaluation process should have adequate knowledge of the program. The stakeholders who will involve in the evaluation are nutritionists, providers, funding agencies, and participants. The data will be communicated to stakeholders through reports.
