Leaders drive programs that integrate patient and family participation into all facets of treatment and provide a welcoming atmosphere for medical professionals as they acclimate to new methods of working.

The inclusion of family members in the care process is another strategy. Health professionals should recognize the value of family members as participants in the care and actively urge patients to include them in their treatment. One other way to enhance collaboration is to empower and motivate personnel. Healthcare institutions strive to get the buy-in of their workers and clinicians. Leadership within an institution may achieve this by continually stressing the significance of particular undertakings. Leaders need to encourage personnel to discuss the benefits of patient and family participation during staff meetings.

Change Management

Organizations in the healthcare sector are evolving at a rapid pace. As care coordinators, we have an ethical obligation to serve all patients fairly and without bias. The ability to deliver treatment that results in excellent outcomes and patient satisfaction depends on our ability to put these fundamental ethical principles into practice.

Owing to healthcare policies, patients may now choose from a wider range of alternatives and get safe and quality care. It is our duty as healthcare professionals to see that healthcare laws are implemented properly and that the clients who need them get the treatment they deserve. The Patient Protection and Affordable Care Act (ACA) is a government policy that has had a big influence on healthcare since it was passed into law. Thanks to this act, people may acquire health coverage at a lower cost. Nurses have a powerful role in the coordination and continuum of care. All nurses must be cognizant of the care coordination process and how safety, ethics, policy, physiological, and cultural needs affect care and patient outcomes. As a nurse, care coordination is something that should always be considered. Nurses must be aware of factors that impact care coordination and of a continuum of care that utilizes community resources effectively and is part of an ethical framework that represents the professionalism of nurses. Understanding policy elements helps nurses coordinate care effectively.

This assessment provides an opportunity for you to educate your peers on the care coordination process. The assessment also requires you to address change management issues. You are encouraged to complete the Managing Change activity.

Preparation

Your nurse manager has been observing your effectiveness as a care coordinator and recognizes the importance of educating other staff nurses in care coordination. Consequently, she has asked you to develop a presentation for your colleagues on care coordination basics.
