Medical errors have been rampant in almost all healthcare systems globally. As a result, most healthcare organizations in the United States have tried to implement measures to curb the prevalence of medical errors. That is because most healthcare providers know that mistakes happen and, therefore, need to ensure that effective strategies are implemented to enhance patient safety and accuracy in patient care. Most healthcare organizations then tend to ensure that they design quality improvement programs that facilitate the change. The presentation will evaluate the current quality improvement initiative in a healthcare setting, what prompted its implementation, problems encountered, the initiative success benchmark, core performance measurements, quality indicators impact, interprofessional perspective, and recommend additional indicators.

Our healthcare organization serves a diverse population, thus making engaging in different quality improvement projects necessary to increase the institution’s healthcare efficacy. For so long, the healthcare management received complaints concerning medical errors that deteriorated the patient’s perception of the healthcare organization. In most of these cases, the healthcare professionals were unaware that the patients had been attended to by other healthcare professionals, thus complicating the treatment and disease management process. As a result, the nurse manager and the other interprofessional teams proposed incorporating an electronic health records (EHRs) system. According to Colquhoun et al. (2020)The Data Analysis And Quality Improvement Initiative Proposal Paper, EHR is a crucial healthcare quality improvement technique that helps healthcare providers detect instances of medical errors since it supports alerts that notify the healthcare team of drug or food interactions that may harm the well-being and health of the patients.

The main problem encountered after EHRs implementation concerns inadequate training for some healthcare providers. As a result, some healthcare providers did not have enough knowledge, especially in retrieving and sending data, since, during training, the trainers did not evaluate if the healthcare providers understood how the EHRs workflow operates (Liss et al., 2020). As a result, most healthcare providers continue documenting and charting patient data using traditional paperwork, leading to the continued encounter of medical error instances in the healthcare organization. Generally, these instances need to be curbed as soon as possible since they risk patients’ safety and the healthcare organization’s sustainability and competitiveness. That is because most patients may be prompted to seek healthcare services from competitors. The other issue concerned privacy, especially regarding the system’s passwords, since healthcare has also experienced high nurse turnover. This situation increases the risk of patient information being shared with a third party by malicious individuals, which may trigger legal issues for healthcare and its team. Therefore, designing effective workshops for healthcare providers will be necessary.

Despite the different challenges the EHRs system faces in healthcare, it has some success benchmarks. For instance, despite the healthcare providers finding it hard to send and retrieve data, the EHRs can easily research drugs, their side effects, and contradictions. Also, healthcare has improved medication prescription since healthcare providers, especially pharmacists, can easily formulate health plans for standard and approved dosages and specific contraindications and administration. Also, those healthcare professionals utilizing the system can use the split screen that helps compare pre-admission medication to identify potential adverse reactions and interactions. That is especially when administering new drugs to the patient. Hence, the healthcare organization is quite experiencing an improvement in prescription due to the presence of electronic prescribing compared to manual, which increases convenience between patients and providers.

For healthcare outcome improvement to be noted, effective outcome measurement is necessary. According to Litwin (2021), the main aim why healthcare conducting performance measurement of a quality improvement project is to ensure that it achieves the healthcare’s Quadruple Aim. The Quadruple aims to enhance patient care experience and population health and reduce healthcare per capita cost and staff and clinician burnout. Therefore, the core performance measurement for EHRs in our healthcare organization concerns patient safety and patient experience. In that regard, patient safety assessment will evaluate the reduction of medical error in the healthcare organization, including the main patients affected, to help form strategies to curb the errors. Also, concerning patient experience helps gauge the patient attitudes and beliefs of the care provided in the healthcare organization to evaluate patient satisfaction levels (Litwin, 2021)The Data Analysis And Quality Improvement Initiative Proposal Paper. The healthcare’s quality indicators include patient quality indicators that provide important information that indicates the avoidable safety events that can enhance care delivery. Also, the prevention quality indicators enable the healthcare to carry out follow-ups, especially after the patient is discharged or in the case of an outpatient case, to ensure that the patient recuperates easily

Quality Initiatives efforts have to be subjected to a clear definition to meet the needs of the healthcare organization issue. In that case, there is a need to involve the multidisciplinary team to help make diverse but effective approaches toward the initiative implementation (Ribeiro et al., 2019). For instance, for healthcare organizations that integrate nurses and other healthcare providers like social workers, there is always a need to ensure that each team is equally represented in the project team. The reason for such considerations concerns each team’s different roles in the healthcare setting. For instance, for medical errors in prescription, all healthcare providers, from primary care providers to pharmacists, must be integrated into the training process to understand the EHRs processes and workflows. Such training will be effective since it will enhance interoperability and interprofessional collaboration. Also, it will cultivate respect for each team’s role in the patient treatment plan.

The increasing quality measurement interest is supported and accompanied by the growing ability of healthcare organizations to measure and analyze care quality. Hence, the other recommended indicators that healthcare needs to assess are the structural quality indicators, like the sufficiency and appropriateness of equipment and facilities, medical staff qualification, and staffing ratio can help assess the efficacy of EHRs in reducing medical errors (Christensen & Laegreid, 2020). Process indicators are employed to determine if or not decisions displaying higher care are obtained during the delivery of services. Process indicators should preferably be based on credible scientific evidence that complies with these metrics for improved care outcomes. In conclusion, for our healthcare organization EHRs initiative to work better, we need to expand the current two indicators on patient safety and the prevention quality and incorporate other process and structural indicators to ensure that every action follows an evidence-based approach The Data Analysis And Quality Improvement Initiative Proposal Paper.
