General Guidelines for Comprehensive Case Study – Week 7 Assignment 1 (Tommy Acker, Child Abuse)

Your case study should not be just a SOAP note of a patient encounter—it should be a comprehensive evaluation of an iHuman patient case.

You will complete a Comprehensive SOAP note on the approved Pediatric SOAP Note template based on the findings of the iHuman case – then major research and discussion of the disease process are presented.

Tommy Acker Case

This is a case involving 26 months male child who has been diagnosed with Down syndrome. The child is called Tommy Acker. After having a nap, the child started showing symptoms of being resistant to eating and drinking. The symptoms that have been revealed by Crombag, Boeije and Iedema-Kuiper (2016) for a child suffering from Down syndrome includes the fact that the child as slanting eyes and there is a fold in the skin coming out of the child’s eyelashes. Additionally, a child with Down syndrome has symptoms including having a flattened face as well as the nose and having a small head and ears (Horvath and Franceschi, 2015). This indicates that that the child has the conditions and hence there is a need for the analysis to be undertaken in the management of the condition that the child is suffering from.

The cases of Down syndrome may end up leading the increasing cases of child abuse. As such, Pearce and Pezzot-Pearce (2013) assert that the child with fear and lacking social participation indicates that there are possible cases of mistreatment. Additionally, the indications of child abuse include the case where a child has bruises of injuries resulting from the abuse. However, there is a need for an analysis to be undertaken to ensure that the child has probably been abused. These start with the fact that one has to befriend the child and hence gain their trust. Furthermore, there is a need for the analysis of the radiographic evidence for any fractures that occur as a result of any possible abuse. In this regard, the child will have to take the initiative of ensuring that there is a way of managing the cases of abuse. Along with this, there is a need for the management of the childhood issues and the parents have an obligation of assessing the possible resulting issues from the abuse. This will help in ensuring that the children with Down syndrome are not neglected and hence end up feeling unwanted in the society. Biehal, Sinclair, and Wade (2015) as well argue that the physician plays a crucial role in the management of children who have been abused in the community. Besides, Talley (2014) agrees that the living standard of the children with Down syndrome will highly depend on the management that the provided by the physicians. This is attributed to the fact that eh physician can be able to offer support in relation to the identification of the challenges that are faced by the child and the possible solutions. In addition, the physician plays a critical role in advising the parents of the children on the ways of managing the children when issues regarding abuse occur. This will help in improving the living standard of the children.
